In this simple application process we require four things:
1)  Your name, mailing address, phone numbers, and e-mail.
2)  Child's name, age, diagnosis, and brief explanation of their
      situation.
3)  Your most immediate need or obstacle to be addressed.
4)  What other assistance/aid have you received or are
      currently receiving.
